Investigation Summary
At approximately 5:15 p.m. on April 6, 2006, Employee #1, a card machine operator, was involved in the manufacturer of various types of polyesters fiber fillings. He noticed that some of the fibers, that get air-borne during normal operation, were accumulating on the motor and the belt pulleys. The motor and belt pulleys were mounted to the frame of the card machine and were at floor height guarded behind doors that were interlocked with the electrical system. He was trying to remove the fibers with out locking out the machine and had done this in the past. He got down on the floor on his hands and knees and crawled through an opening to gain access to the motor and the belt pulleys. His right index finger was caught in the pinch point of the belts and pulleys. He was hospitalized with an amputated right finger.
