Investigation Summary
At approximately 3:30 p.m. on April 7, 2006, Employee #1 was lifting a 30-lb pallet that had fallen between the mast and supporting post of a forklift at Packaging Advantage Corporation. He was injured, when the pallet fell onto the tip of his left pinky finger. He was treated at Technimed in Vernon, for a severe laceration to his finger.
