Investigation Summary
On October 18, 2005, Employee #1, with Euro Coffee (a retail gourmet coffee business) was in a storage area retrieving some cardboard boxes to fill an order. A coworker was operating a forklift, also retrieving cardboard boxes. While backing up, the forklift operator inadvertently struck a free-standing stack of pallets, causing the top two pallets to fall. Employee #1 was struck by the falling pallets and boxes. He was hospitalized at County USC Medical Center for treatment of leg fractures, including surgery. The investigation determined that the accident was caused by stacking of pallets and boxes to an unsafe height, rather than utilizing storage racks or other similar means to prevent them from tipping and falling during the forklift operation.
