Employee Falls and Fractures Leg

Employee #1 of CMI Freight and Transport was transferring trash from a trash bin to a large open-top, 20-foot container. He climbed into the smaller trash bin while elevating it with a forklift. Apparently, either the movement inside the trash bin or the poor condition of the forklift might have caused the trash bin to slip off the forks to the ground. Employee #1 fell to the ground and the trash bin landed on his left leg fracturing it. He was admitted to the hospital. Although the bin was designed to be lifted with the forklift, the trash bin had not been secured to the forks or mast to prevent it from tipping, slipping or falling.
