Investigation Summary
At approximately 11:00 a.m. on May 26, 2005, Employee #1 was working at the construction site near a dormer window located on the second story of a single-family residence. Employee #1 refused to be interviewed and there were no direct witnesses to the incident, and it could not be determined precisely what he was doing at the time of the incident. According to the project supervisor Employee #1 was most likely either removing wood siding or nails from the frame when he slid off of the roof falling 11 ft 5 in. to a concrete slab. The pitch of the roof where Employee #1 was working is 9-to-12. Based on slide marks visible in photographs taken by the employer, Employee #1 appears to have been on the roof to the right (facing the window) side of the window when he fell. Employee #1 was not using fall protection when he fell and suffered fractures. Employee #1 was hospitalized.
