Investigation Summary
At approximately 4:00 p.m. on May 12, 2003, Employee #1 was painting the outside of a building from a 20-foot metal extension ladder. Employee #1 lost his balance which caused the ladder to shift. Employee #1 fell from the ladder approximately 15 feet to the ground and fractured his right wrist and left leg. The injuries required hospitalization.
