Investigation Summary
At approximately 2:15 a.m. on March 30, 2005, Employee #1, a warehouse team leader with Whole Foods Market, located in Vernon, California was performing his duties of employment. Employee #1 and a coworker finished loading the trailer, Number 5354 of a Freightliner tractor, trailer truck cab Number 355030, at one of the facilities loading docks. They were preparing to embark on their delivery run. After the coworker pulled out of a docking bay, Employee #1 noticed that the trailer's lift gate was still in the down position. The coworker drove the vehicle away from the dock. He stopped, with the truck's engine still idling in his usual location at the parking lot, so that the lift gate could be raised during the pretrip inspection of the vehicle. When both Employee #1 and the coworker started to exit the vehicle, Employee #1 slipped on the floor of the cab near the edge. He fell approximately 4 ft to the ground, striking the back of his head on the truck step. Employee #1 sustained concussion to his head. The coworker discovered Employee #1 on the ground after he circled the vehicle and was walking around the back side of the trailer. Employee #1 was wearing work boots with slip-resistant soles at the time of the incident. Employee #1's direct supervisor was not on site at the time of the incident.
