Employee Amputates Finger in Crane Operation

At approximately 5:45 a.m. on March 16, 2005, Employee #1, a production manager for a company that rebuilt and sold electric motors, was using a Shepard Niles, 3 ton, overhead crane, Serial Number 3008, to hoist an electric motor rotor, weighing approximately 1,500 lbs. In order to lift the rotor, a 20-inch lifting collar needed to be placed around the shaft of the rotor and a locknut screwed on to the end of the shaft to hold the collar in place. Employee #1 grasped the crane's controls in his right hand and grabbed the lifting collar in his left hand to steady the load. He began hoisting the load. As the lifting collar reached the locknut, Employee #1's left index finger became caught between the lifting collar and the shaft locknut, which lead to the amputation of his left index finger at the base of the fingernail. The accident investigation determined that the accident was caused by Employee #1 placing his hand in an area where the collar and locknut came together when the rotor was hoisted.
