Investigation Summary
At approximately 10:30 a.m. on July 13, 2004, Employee #1, a warehouse worker with 25 years of experience, was on a Pallet Jack, Model Number NPP60, Serial Number 2HL05496, rearranging merchandise on a storage rack approximately 5 feet high. At the time of the accident, he was pulling a box when he lost his balance and fell to the concrete floor, breaking his ribs on the right side, his right shoulder, and his vertebrae. He was transported to Saint Francis Medical Hospital where he was admitted for approximately 10 days. Further investigation revealed that he did not have any fall protection while on the pallet jack.
