Investigation Summary
At approximately 2:30 p.m. on July 1, 2004, an employee who was a line worker at Overhill Farms, frozen food processor and cold packer, placed rolls of dough at the input end of a tortellini machine to make tortellini. As the dough made its way through the machine, it began to jam up and accumulate at the rotating blade mechanism at the rear of the machine. The employee stopped the machine and reached from the rear of the machine with his left hand to unjam the dough from the blade area. At the same time, a maintenance worker who was standing adjacent to the employee inadvertently turned on the machine, resulting in the employee's fingers becoming caught in the rotating blade mechanism. The employee was transported to TechniMed, and then he was referred to Orange County Surgical Center Hospital, where he was hospitalized and treated for amputation of his left middle and ring fingers.
