Investigation Summary
Employee #1, a mason tender, was using a material hoist arm attached to a fabricated frame scaffold to hoist a 5-gal-bucket of mortar to his coworkers, who were on a scaffold two frames high. As Employee #1 pulled on the rope to raise the bucket, the hoist became detached from its attachment point on the frame and tumbled toward the ground. Employee #1 attempted to run. However, he stumbled amid muddy conditions, losing his head protection just before the arm struck the back of his head. Employee #1 was taken to the hospital, where he was pronounced dead.
