Investigation Summary

On September 17, 2003, an employee was working as a ride attendant at a carnival. He attempted to secure a patrons seat prior to the ride start up. As the employee attempted to signal a second attendant of the problem, the second attendant misinterpreted the signal and instructed the control operator to start the ride. The employee continued to hold on to the patron's unsecured seat to prevent injury to the patron as it slowly rose to start the ride cycle. The employee finally let go when he reached an altitude of approximately 25 feet. The employee fell to the ride deck below sustaining a severely bruised right foot. The employee was not hospitalized. The interviews revealed that the workers had received training in the safe operation of the ride. Observations from the operators booth revealed the operators view was somewhat blocked in regard of to where the attendant was standing. Interview with the operator revealed that ride interlocks and GO LIGHTS were lit to begin the ride.
