Investigation Summary
At approximately 5:20 p.m. on August 28, 2008, Employee #1 was standing on top of a tanker truck holding a nozzle of a hose. He was transferring a class 1a flammable liquid from another over the road tank truck. The trucks were not bonded together or grounded and both were operating at the time. The nozzle of the hose was not bonded to the tank nor did it extend into the tank far enough to reduce the build up of flammable vapors. Employee #1 had not been trained to handle class 1a flammable liquids and was not aware of the hazards involved. A flash fire occurred and Employee #1 jumped from the tanker. The fire most likely occurred due to a build up of static electricity inside the tank which ignited the vapors at the top of the tank where the vapor to air mixture reached the flashpoint. Employee #1 received first and second degree burns to his lower legs as well as a fractured wrist and cuts and bruises from jumping off the truck. Employee #1 was hospitalized.
