Employee killed after being struck by catepillar

On April 8, 2004, Employee #1 was conducting a soil test on an active excavation site. A coworker was operating a Catepillar 621 Pan. While the employee was kneeling down at the test site, the coworker ran over him with the Catepillar 621 Pan. Employee #1 was killed.
