Investigation Summary
At approximately 8:30 a.m. on November 3, 2003, Employee #1 and a coworker were operating a sawmill lumber sorter and stacker. The coworker engaged the control panel to lower the retaining straps on bin Number 35, reject bin, for the rejected lumber to be sent to the stacker. The coworker observed that one of the straps could be caught by the chains that carry the lumber to the stacker, and attempted to move the bundle of lumber and the retaining straps away from the chains. Employee #1 assisted the coworker by entering the lower level of the sorter at bin Number 35. Employee #1 attempted to manually push the lowered bundle of lumber sideways to avoid the retaining straps from getting caught in the chains. While Employees #1 and the coworker were working on clearing the lumber from bin Number 35, the sorter was operating and ejected a piece of lumber to bin Number 35. The lumber fell from the top of the bin, approximately 13 ft, and struck Employee #1 in the head. He was hospitalized with blunt force trauma to the head.
