Employee killed when struck by lever of jack in the head

At approximately 12:08 a.m. on December 16, 2003, an employee and two coworkers were attempting to remove a well water-pump with the use of a "hand pump-jack". The load, approximately 235 lbs, placed on the lever slid approximately 13 to 15 ft, through the securing chain causing the lever to move upwards with great force, approximately 6000-7000 ft lbs, striking the employee in the head.
