Investigation Summary
At approximately 7:35 a.m. on January 9, 2004, Employee #1 was standing on a horse scaffold to attach a lath on the front side of a one-story home, under construction, in preparation for installation of the facade. The scaffold measured 4 feet in high. For some reason, he fell off the scaffold. Emergency services were contacted and Employee #1 was transported to University Medical Center in Las Vegas, NV, where he was pronounced dead later that evening.
