Employee injured in fall through HVAC duct, later dies

On September 29, 2003, Employee #1 was installing Nelson studs to secure the floor decking on the third level of a building when he fell 14 ft 11 in. through an open HVAC duct and landed on the second floor. He sustained head injuries and was transported to the hospital, where he was placed on life support. He died on October 2, 2003, after being removed from life support.
