Event: 09/29/2003
Carpenter Erecting Scaffold Stair Tower Falls and Is Killed

At approximately 7:30 a.m. on September 29, 2003, Employee #1, working as a carpenter, was erecting a scaffold stair tower. As he was at a height of approximately 59 feet, the stair unit he was installing became stuck and failed to connect into position. Both Employee #1 and the competent person there attempted several times to reposition the stair unit. Employee #1 was attempting one more try, but neither he nor the competent person tied off. When Employee #1 kicked at the base of the stair unit, he missed and fell approximately 59 feet to a concrete sidewalk. His fall was broken when he bounced off the inside of the tower and hit an equipment truck before landing on the concrete sidewalk. He sustained multiple injuries, including head trauma and body fractures. He was killed.
