Maintenance Supervisor Dies after Fall from Ladder

On July 12, 2005, Employee #1, a maintenance supervisor with the Waukesha YMCA, was changing a light bulb in a whirlpool room. He had drained the whirlpool and placed a 10 ft Werner aluminum stepladder in the whirlpool basin. Employee #1 then climbed the ladder and removed the protective light panel. He fell from the ladder and struck his head on the ceramic tile basin edge. Emergency responders transported Employee #1 to Waukesha Memorial Hospital for treatment of head trauma. He died three days later of the head injury and other complications. The investigation determined that one factor may have been that Employee #1 had donated blood earlier in the day before the fall.
