Investigation Summary
On April 2, 2004, Employee #1 was installing an air ride cab suspension system on a truck that required the hydraulic bed to be suspended in order to gain access under the cab. While he was located under the cab, the suspended hydraulic bed failed and fell on Employee #1. He was crushed between the hydraulic bed and the main frame of the cab. Employee #1 died due to injuries sustained.
