Investigation Summary
At about 1:15 p.m. on November 29, 2006, Employee #1 was repairing the external masonry of a library building at the University of California in San Diego, California. He fell 11ft from the eight-floor building roof edge and struck his head. Employee #1 sustained a concussion and was killed. The fall protection system was inadequate, and the arrest was not activated.
