Investigation Summary
A approximately 11:45 a.m. on January 10, 2006, Employee #1 was working as a truck driver for a local trucking company, Chef's Warehouse. He was making a delivery at the Paradise Point Resort in San Diego, CA, when he fell about 3 or 4 feet from the back of a 24-foot International delivery truck. He sustained a fracture to his head and was killed. The medical examiner's report indicated that the cause of death was blunt force trauma to the head with contributing preexisting factors.
