Investigation Summary
At approximately 10:45 a.m. on September 23, 2003, an employee was preparing to paint the cupola located on a 14-in. 12-slope roof, when his foot got caught between the dormer and the chicken ladder located on the roof. The employee fell approximately 11 ft and 8 in. from the eave to the ground level which resulted in his death. The employee died due to multiple injuries caused by the fall.
