Employee dies from 15 foot fall from ladder

On September 22, 2003, Employee #1 was actively climbing a ladder that was not properly secured. He lost his balance and fell approximately 15 feet to the ground. Employee #1 was hospitalized, where he later died after being in a comma state for eight days.
