Investigation Summary
On November 21, 2003, Employee #1, working as a pipe layer, was assisting with pipe positioning, when it was discovered that the pipe didn't fit the area that was dug out for it. The area needed to be dug out at the sides of the pipe. It was revealed that the employee climbed into the pipe joint, which was already laid to wait, while the backhoe operator dug along the sides of the excavation to widen it. Employee #1 was struck by the bucket as he attempted to exit the pipe. There were no eye witnesses. The backhoe operator said that when he swung the bucket around after laying the pipe on the bank, he saw Employee #1 slumped over inside the pipe. Employee #1 died as a result of his injuries.
