Employee Dies in Trench Collapse

On April 5, 2005, employees of Utility Contractors of America, Inc. were working in a trench thirty-three feet deep. The employees entered the trench approximately two hundred feet from the trench box and walked toward the box, with fifteen to twenty feet separating each of them. The trench wall collapsed, and Employee #1, who was farthest from the trench box, was trapped and killed.
