Investigation Summary
On September 17, 2003, Employee #1 was helping to load a mud motor used for directional boring onto a flatbed trailer. The trailer was already loaded with four secured stacks of bore rods, and the mud motor was going to be loaded on top of the rods. Two 4 ft long by 4 ft wide pieces of wood were placed on the stacks, and the mud motor was going to be placed on the wood and then secured. The motor had been placed on the wood when it began to roll toward Employee #1, causing him to fall from the trailer to the ground. The motor also fell, striking him in the back as he lay on the ground. Employee #1 was flown by medical helicopter to the hospital, where he died the following day.
