Employee is injured when struck by rebar

Employee #1 was loading 0.625-inch rebar into a MEP rebar bending machine. He could not feed the rebar into the first pulley so he re-bent the end of the rebar. He again tried to feed it into the first pulley and it still would not fit. He then loosened the pulley to feed the end of the rebar into the first pulley. The employee then went to the control panel to feed the rebar to the second pulley. He thought he turned the "in" switch but the rebar popped out of the pulley and struck him in the mouth, knocking out a front tooth on the lower right side of his mouth.
