Investigation Summary

On September 17, 2003, Employee #1 was packing a bundle of roofing as part of a job to remove an old roof and install a new one. He was walking across a portion of the old roof when he stepped on a wet shake and slipped. Employee #1 fell to the ground, landing on his head and on the back of his shoulders. He was knocked unconscious and he suffered internal injuries. A coworker began providing first aid and was calling for an ambulance when another coworker also fell from the roof. Both workers were transported by ambulance to the hospital; Employee #1 was admitted and the coworker was treated and released. The roof surface was damp from recent rains and morning dew.
