Investigation Summary
A forklift (Lull) was being used to pass a mortar tub containing approximately 1 yard of concrete through a fifth-level window opening of a five-story building. The forklift operator positioned the mortar tub through the window opening where the concrete was shoveled from the tub into several 5-gallon buckets for transport to the roof level where the concrete was to be used for the construction of a pad in a mechanical equipment room. While employees were shoveling the concrete from the mortar tub, two pieces of wood, approximately 2-in. by 10-in. by 8-ft and 1-in. by 83-in. by 27-in. respectively, that were previously nailed together and secured over a precast window sill, to protect the sill during construction, came loose and fell from the building. The falling wood struck and killed Employee #1 who was working in the area below.
