Investigation Summary
At approximately 2:45 p.m. on December 16, 2003, an employee was nailing the felt paper on the third roll up from the eave when he stepped on the felt paper causing it to tear and the employee lost his balance. The employee, trying to retain his balance, fell over the slide guards onto the lower lever. As the employee was falling, he was observed grabbing onto the slide guard, but was unable to hold on. The employee fell 10 ft 7 in. to the lower level. No one observed the employee as he landed, but was observed laying on concrete after the fall. The employee sustained a bump on the back of his head and requested to get up, but was told to remain lying down until emergency help arrived. The employee was transported to the hospital, where he was admitted with a skull fracture and subarachnoid hemorrhage. On December 17, the employee developed a hematoma. On December 18, the employee went into a coma and under went surgery. The employee never recovered and died on December 19, 2003.
