Investigation Summary
On December 18, 2003, Employee #1 was installing felt paper on an exterior 1-to-12-pitched roof inside of a 4.5-ft-high parapet wall. He had put on a safety harness and lanyard, but failed to tie off to the installed ropes on roof areas. The felt paper he was standing on ripped and caused him to loss his balance and fall 54-ft from the roof. He struck the ground sustaining traumatic injuries that were fatal.
