Employees' fingers caught in card machine

On October 25, 2003, Employees #1 and #2 attempted to remove the choke on a card machine using a wire brush. In both incidents the machine was not shut down and locked out. Employee #1's hand received injury that resulted in bruising and abrasion of his fingers. Employee #2 received injury to his hand that resulted in amputation to his fingers.
