Employee is killed when struck on the head

On October10, 2003, Employee #1 was unloading a 900 pound burial vault lid using a vault dolly. The dolly tipped over, and the motor section struck the employee on the head. Employee #1 was taken by helicopter to the hospital and died the next day.
