Investigation Summary
Employee #1, a supervisor, and a coworker, a laborer, were installing storm drain pipe in a trench approximately 9 feet deep. The two employees were unhooking the last piece of pipe that had been lowered into the trench by a track hoe bucket. As the pipe was lowered, the east wall of the trench collapsed. Employee #1 was completely buried by the collapse and was killed. The coworker was struck in the leg but managed to exit the trench. The investigation revealed that neither a trench shield nor other type of protective system was used in the trench which had near vertical walls. In addition, the trench was subject to vibrations from equipment operating nearby (a track hoe, a skid steer forklift, and street traffic).
