Investigation Summary
Employee #1 was dismantling the big top the day after the last performance of a circus. A forklift was used to hold a section of steel in place while it was unfastened from the rest of the structure when a section of the structure fell onto the employee's left foot. Three toes on his left foot were amputated and he was hospitalized. He was not wearing steel toed shoes at the time of the accident.
