Investigation Summary
On July 3, 2003, Employee #1, a yard foreman of a roofing contractor, was engaged in the demolition and replacement of old wooden vertical support beams with steel I-beams. The beams were used to support the roof of a covered open-sided storage area located in the roofing contractor's company yard. A witness stated that Employee #1 used a metal extension ladder which he leaned against the I-beam approximately 16-feet above the concrete floor. Employees stated the top rung of the ladder was resting and centered on the I-beam and that the top of the rails of the ladder were straddling the I-beam and unsupported. They stated the base of the ladder was at the proper angle and resting in compacted soil away from the concrete pad. They also stated that the ladder was not equipped with any special attachments for single support and that the ladder was not strapped or tied to prevent shifting or racking. An eyewitness stated Employee #1 was working with his feet on the ladder at a height of approximately 10 to 11 feet above the ground. The witness also stated he observed Employee #1 hammer a carriage bolt through the top beam plate and the wood ridge beam. He then observed Employee #1 reach to the right of the ladder and around to the opposite side of the I-beam and ridge beam to attach a nut to the carriage bolt. As Employee #1 did this, the top of the ladder twisted and Employee #1 fell with the ladder to the concrete striking his back and head. Employee #1 sustained severe head and internal injuries and was transported to and died at Suburban Hospital in Bethesda, Maryland.
