Investigation Summary
On February 3, 2004, Employee #1 was on site at the Penske Yard, returning a rental trailer. He had his personal company's semi-tractor cab in neutral, still running, and had the cab on a slight decline. While Employee #1 was hooking the grab handles to the back of the cab, it started to move forward. His foot was caught under the driver side rear dual wheels. Neither the cab's brake system was engaged, nor the wheels were chocked.
