Employee caught in machine shaft

On December 10, 2003, Employee #1 was lubricating the briquetter, while the machine was running. His clothing got caught by projecting bolts on couplings of the counter-rotating drive shafts, pulling him between and around the shafts. Employee #1 right lower arm was amputated and abdominal organs eviscerated. He was pronounced dead, on the scene, by the coroner's deputy.
