Investigation Summary
At approximately 9:50 a.m. on October 22, 2005, Employee #1 was unloading a truck with a standup Raymond forklift. Employee #1, with a load on the forklift was traveling in an aisle in a southerly direction facing the direction of travel with the load behind him. The aisle was approximately 11-ft wide, well lighted with a flat floor in good condition. A collision occurred with another standup Raymond forklift traveling in the same direction with the forks positioned behind. Both employees had been employed by the above company for several years, had been trained on the forklifts, and had no previous incidents involving unsafe usage of the vehicles. Employee #1 stuck the fork of the second forklift. Employee #1's leg was seriously injured. Employee #1 lost a lost of blood and was transported by ambulance to the hospital. At the hospital, Employee #1 went into surgery where his leg was amputated. At approximately 12:30 p.m. on October 26, 2005, Employee #1 was pronounced dead.
