Investigation Summary

Investigation Nr: 200650844
Event: 10/07/2003
Employee is killed when run over by concrete truck

On October 7, 2003, Employee #1, with The Boldt Company, was finishing a concrete sidewalk. Nearby, a concrete truck from County Material Corporation was waiting to be called to dispense. The driver was building air pressure in the brakes by increasing the speed of the truck's engine. When the crew signaled for more concrete, the driver put the truck in forward gear without waiting for the engine speed to return to idle. The truck lunged forward, running over Employee #1, and killing him.
