Investigation Summary
On July 8, 2003, an employee of Pratte Building System was standing adjacent to the east side of a house under construction and was not wearing a hard hat as he was waiting to be evaluated on operating a Gradall forklift. Employee #2, the Gradall operator, did not require the employees to wear a hard hat. While stocking lookouts and outriggers, parts of the facia for the overhang of a two story house, one 2-in by 4-in by 3-ft long wooden board fell and struck Employee #1. He sustained a laceration to the top left side of his head requiring 12 stitches. He was not hospitalized.
