Investigation Summary
Employee #1 was under a trailer, possibly setting up a jack to change a tire on a mobile office trailer. The trailer was attached to a truck. The truck engine was idling in advance of being driven to deliver the trailer. The truck driver pulled forward, unaware that Employee #1 was under the trailer. Employee #1 was crushed and killed when three wheels on the left side the trailer ran over him.
