Employee killed when struck by flying pulley

On June 17, 2003, an employee and four coworkers were using the hoist on a mobile home to pull a stump. The wire rope traveled over a pulley that was supported by brackets. The brackets broke as the employee was beginning to release the pressure in order to change the trucks location. The bracket struck the employee and killed him.
