Investigation Summary
Employee #1 was delivering a 16-inch bit to another employee at a construction site at a private residence. When he arrived at the site, he discovered that the company's 1994 Ford dual-wheel truck was stuck in the mud. A coworker attached a towing strap to a Caterpillar bobcat and the truck and pulled the truck free as another coworker steered the truck. Once the truck was free, Employee #1 disconnected the towing strap from the bobcat and then went under the truck to disconnect the strap from the axle of the truck. The coworker in the truck did not see Employee #1 under the truck and as he began to drive the truck forward, Employee #1 was run over and crushed by the truck. He was transported to a local hospital where he later died from his injuries.
