Investigation Summary
On June 4, 2004, Employee #1 was operating a Stihl gasoline-powered cut-off saw (Model Number TS 760) with a 16-in.-diameter blade to cut a 24-in.-diameter concrete pipe. This dry-cutting was being conducted at ground level while the pipe was braced by lumber. Employee #1 was wearing all necessary PPE (safety shoes, hard hat, hearing protection, eye protection, a dust mask and gloves). After cutting the top of the pipe, Employee #1 began cutting along the side of the pipe. When the pipe pinched together, the saw kicked back and struck his face. He was hospitalized for facial lacerations. The saw was equipped with a blade guard as required and supplied by the manufacturer. Employee #1 had been trained to operate the saw and the employer has a comprehensive safety program.
