Car Falls Off Lift and Fatally Crushes Employee

Employee #1 was on scheduled vacation, but entered his work facility on his own time while the facility was closed and used the company forklift to repair his own vehicle. The vehicle fell off the forks of the forklift, and crushed Employee #1. The company was not aware that Employee #1 was at the facility and that he was performing maintenance work on his personal vehicle. This incident was found to not be a work related accident. Employee #1 was killed.
