Investigation Summary
On June 2, 2003, the employer of Employee #1 was preparing to raise a wooden beam and place it on a concrete column with its south end at the end of a short wall. The beam was 23 ft 4 in. long, 12.5 in. wide on each side, approximately 14 ft 0.5 in. high and weighed at least 600 lb. The north end of the wooden beam was to be adjacent to a cinder block wall. A fiber lifting belt had been wrapped around the wooden beam and tied so that it could be used for connecting to the lifting hook of a 1-ton chain fall. The one 1-ton chain fall had been secured to the eye of the eye bolt. A coworker was next to the south wall at the fire place operating the chain fall. Employee #1 was in the northeast corner of the room by the east wall. The wooden beam had been raised to a height of approximately 11 ft to 12 ft above the concrete floor. When the wooden beam and lifting equipment began to fall, the beam rode down, thus the north end of the wooden beam came close to the northeast corner where Employee #1 was standing. He tried to escape from the corner, but was caught by the falling wooden beam. Employee #1 was killed.
