Employee injured in fall from catwalk

At approximately 8:45 a.m. on June 27, 2003, Employee #1, a temporary worker who was an electrician's apprentice, and a coworker were on a 4 ft wide catwalk in an attic, putting together a fluorescent light fixture. The catwalk spanned several wood trusses that were between 21 1/2 in. and 24 in. apart. Employee #1 was trying to step around his coworker when he walked off the catwalk onto insulation that was stapled between the two trusses. It had no drywall supporting it underneath and he fell 16 ft onto the concrete floor. Employee #1 sustained a fractured skull, a fractured spine, and several contusions. The catwalk did not have guardrails, and neither Employee #1 nor his coworker were using any form of personal fall protection equipment. The employer was unable to verify the adequacy of the employees' fall protection training, failed to identify specific fall hazards at the site where fall protection was required, and did not ensure that its employees were using fall protection while working in the attic.
